WHO MAY AVAIL OF THE SERVICE:

Persons, natural or juridical, who would like to avail the service.

MUST READ:

WHAT ARE THE REQUIREMENTS:

Verification of 12.50 Elevation

  1. Duly accomplished Application Form for Verification of the Reglementary 12.50-meter Elevation.
  2. Location/ Lot Plan with Technical Description duly signed and sealed by a Licensed Geodetic Engineer.
  3. Vicinity Map including coordinates of the subject area.
  4. Authorization Letter (if representative).
  5. Photocopy of valid ID.
  6. Survey Fee.

NOTE:

The original copies of foregoing requirements shall be presented during submission. However, if the original copy of the document is not available, a certified true copy thereof from respective government agencies should be presented. Submit all requirements in TWO (2) COPIES fastened in LONG WHITE FOLDERS.

  • If subject lot is above 12.50 meters elevation, recommends approval of Certification and
    proceed to next step.
  • If subject lot is below 12.50 meters elevation, prepares Notice of Denial.

NOTES:

  1. The foregoing process applies to application with complete requirements
    and without pending case.
  2. Technical Services Requisition Forms are available for download (please see top of this page) or at the frontline desk.
  3. In the How to Avail the Service section, Duration refers to the time allotted for each activity, excluding queuing time. Processing Period refers to the time from the acceptance of duly accomplished application form up to the approval of the same, including queuing time.
